Transition Co-ordinator

Overview

Students with individual educational needs who are in Year 11 and their parents are supported by a transition coordinator in exploring post-secondary opportunities. The coordinator attends individual educational programme meetings, prepares action plans, organises orientation visits, and ensures the best educational or employment options for each student. 

What you'll get

  • An individual transition plan for learners with a statement of needs / attend a CCP class who are in Year 10 and Year 11 is developed and implemented by the Transition Coordinator, through observation sessions, and meetings with the learner, family and the school team.
     
  • The process starts during the learners’ IEP meeting in their last year of secondary school (Year 11) whereby their parents/family members, the Head of Department (Inclusion) and the school’s Senior Management Team are also present to discuss the learners’ best possible options for their future.
     
  • Throughout the scholastic year (Year 11), sessions are held individually with the learners to discuss their best possible options. Other meetings are held at the entities chosen by the learners and their families, and at the preferred post-secondary provisions. Small groups of learners participate during these visits to ensure that queries and concerns regarding their future are tackled appropriately.

Eligibility

Secondary school learners who can benefit from this service include those who:
 
  • are in Year 10 or Year 11;
  • have a statement of needs and have the support of a Learning Support Educator (LSE);
  • have learning difficulties and/or additional needs;
  • have access arrangements.

The service is offered by NSSS through referrals from schools.
