Food Supplements/ Foods for Special Medical Purposes (FSMP) Notification Form

Overview

This form must be submitted when a notifier wants to place a food supplement/FSMP on the local market. The notifier must submit his/her contact details as well as the information related to the food supplement/FSMP. The product label together with proof of payment must also be submitted.

What you'll get

Once the form, together with required documents, have been submitted, all documents are reviewed. If the submitted documents are correct, a notification receipt is issued. The process takes a minimum of 2 weeks.

Eligibility

This form needs to be filled in by a notifier when a food supplement/FSMP needs to be placed on the local market.
