This is an application for the purchasing of a grave in any Government Cemetery in Malta and Gozo.

People or companies having a cooling tower system and/or evaporative condensers are obliged by law to register with the Environmental Health Directorate by filling in this form.

This form must be submitted when a notifier wants to place a food supplement/FSMP on the local market. The notifier must submit his/her contact details as well as the information related to the food supplement/FSMP. The product label together with proof of payment must also be submitted.

All swimming pools are required by law to be registered with the Environmental Health Directorate so that they may be inspected from time to time for their compliance.
